How Much Is Dangerous

Ingestion of as little as 0.2% of a cat's body weight in plant material can produce serious toxicosis, and in practical terms, a fairly small number of leaves is enough to cause significant illness in most cats — this is not a plant where "just a nibble" can be assumed harmless.

Symptoms

  • Drooling and vomiting, typically beginning within a few hours of ingestion
  • Diarrhea
  • Weakness and lethargy, sometimes progressing quickly
  • Abnormal heart rate — notably, this toxin classically causes a slow heart rate (bradycardia) rather than the rapid rate seen with many other cardiotoxic plants, which is a distinguishing clinical clue
  • Low blood pressure
  • Muscle tremors and, in severe cases, seizures, reflecting the toxin's broad effect on excitable tissue beyond just the heart
  • Collapse or coma in severe poisoning

Time to Symptoms

  • Do not induce vomiting at home without veterinary guidance, particularly given the cardiac component that benefits from monitored, clinical decontamination
  • Do not assume honey (including "natural" or farmers'-market honey from an unknown source near azalea/rhododendron plantings) is automatically safe for any household member, human or feline, given the documented "mad honey" phenomenon

Veterinary Treatment

Decontamination if caught early in the course, IV fluids, and supportive cardiac monitoring and management — medications to support heart rate and rhythm as the specific pattern requires, which given the bradycardia this toxin classically causes, may include atropine to raise heart rate rather than the rhythm-slowing drugs used for some other cardiotoxic plants. Most cats recover with prompt, appropriate supportive care, though cases with significant cardiac involvement require intensive monitoring until the toxin's effects have fully resolved.

Grayanotoxins, a family of compounds that bind to sodium channels in cell membranes throughout the body and lock them in an open, activated state, disrupting normal electrical signaling in heart muscle, skeletal muscle, and nerve tissue simultaneously — which is why this toxin produces such a broad mix of cardiac, muscular, and neurological signs rather than affecting just one organ system. All parts of the plant are toxic, including nectar, and honey made by bees that fed primarily on azalea/rhododendron flowers has historically caused human poisoning (sometimes called "mad honey"), illustrating how potent and stable these compounds are even after passing through another organism.
